Plötzlicher Epilepsietod – SUDEP („sudden unexpected death in epilepsy“) verstehen und verhindern

SUDEP is the abbreviation used to describe the sudden, unexpected death of a person with epilepsy (PWE) that occurs under benign circumstances and without evident causes of death. One in 1000 PWE dies of SUDEP per year. The most important risk factors include focal to bilateral and generalised tonic-clonic seizures (TCS), nocturnal seizures, poor treatment-adherence and living or sleeping alone. The pathomechanisms comprise a spectrum of cardiovascular and respiratory disturbances that occur in association with or independently of seizures. Recorded sudden death cases suggest that SUDEP is commonly caused by central apnoea with hypoxemia and secondary asystole that occurs in the early postictal phase of a TCS. Improved seizure control and measures of nocturnal supervision reduce the SUDEP risk. Counselling about SUDEP has no sustained negative impact on PWE or their relatives. All PWE and their relatives should be informed about SUDEP and measures to reduce the risk.
