<p>Most patients with Parkinson’s disease experience fluctuations in the severity of their motor and non-motor symptoms. Levodopa-related fluctuations include oscillations in the medication response of both motor or non-motor symptoms and can have significant impact on function and quality of life. This review provides a&#xa0;practical summary of the features of medication-related fluctuations and describes therapeutic approaches to their management. In addition to these types of medication-response fluctuations, many patients may have multiple variations in the severity of their symptoms, which can be triggered by a&#xa0;variety of factors not directly related to their medication. For these we propose the term of spontaneous state variations and discuss the clinical implications of this distinction. Thus, this review provides a&#xa0;comprehensive overview of the different types of periodic symptom changes in Parkinson’s disease and their practical significance in the clinical context.</p>
