Background <p>Various sperm selection methods are used in ART to isolate high-quality spermatozoa. Traditional methods like DG, Swim up, &amp; latest techniques like Microfluidics are used. Centrifugation is an important part of DG and Swimup, leads to iatrogenic oxidative stress, increased DNA fragmentation and impaired sperm function. Microfluidics select high-quality spermatozoa, but expensive, low yielding and has limited clinical translation. Antioxidant supplementation to semen processing media might be less damaging, less expensive and more affordable to improve clinical outcomes.</p> Purpose <p>The aim of this scoping review (registered by Open Science Federation (ID: ytq36)) is to analyse evidence and find research gaps in studies which have used antioxidant supplementation.</p> Methods <p>207 records in 11 databases were screened according PRISMA-ScR &amp; PICOS techniques. Human Studies in ART settings, utilizing invitro antioxidants with laboratory/clinical reported outcomes were included. Studies based on animal models, in vivo, non-ART settings, and non-peer reviewed literature were excluded. Out of 146 reports screened, 119 were excluded and 27 were included.</p> Results <p>Alpha-lipoic acid, brain-derived neurotrophic factor, melatonin, or L-carnitine supplementation had a significant effect on ROS reduction and DNA fragmentation in sperms but studies on clinical outcomes using these supplements are limited. Quality assessment (ROBINS-I) was moderate overall risk of 67% of the studies; GRADE stated no limitation in publication bias in primary results.</p> Discussion <p>In vitro antioxidants are effective against centrifugation-induced damage, providing a cost-efficient alternative to expensive low yeilding techniques.</p> Conclusion <p>Antioxidant supplements in semen processing media are effective in vitro but would need clinical validation.</p>
