Coffee Consumption and its Association with Knee Osteoarthritis: A Systematic Review and Meta-Analysis of Observational Studies

Knee osteoarthritis (OA) is a major contributor to disability worldwide, and coffee consumption has been proposed as a potential lifestyle factor influencing disease risk. To clarify this association, we conducted a systematic review and meta-analysis of observational studies in accordance with PRISMA 2020 guidelines.
MethodsSearches of PubMed, Scopus, EBSCOhost, and Google Scholar through February 2026 identified three eligible studies, including 40,244 participants (6,505 cases and 33,739 controls). Studies reporting odds ratios (ORs) for knee OA among coffee consumers were included. Pooled ORs were calculated using a random-effects model, and subgroup analyses were performed by sex and daily coffee consumption.
ResultsThe pooled analysis showed no overall relationship between coffee intake and knee OA (OR = 1.04, 95% CI = 0.94–1.14, p = 0.49, I² = 31%). Subgroup analyses revealed no significant association in females, but a modest association among males (OR = 1.16, 95% CI = 1.02–1.33, p = 0.02, I² = 3%), with high consumption (≥ 4 cups per day) linked to greater susceptibility (OR = 1.49, 95% CI = 1.06–2.10, p = 0.02, I² = 21%). Sensitivity analyses confirmed the robustness of these findings.
ConclusionOverall, coffee consumption was not clearly associated with knee OA. Exploratory subgroup findings suggested higher odds among males, particularly at higher intake levels; however, these findings should be interpreted with caution, as the evidence is observational and limited. Future prospective studies using standardized exposure and outcome assessment are needed to confirm whether this pattern reflects a true association or residual confounding.
