A Clinicopathological Study of Vocal Fold Abnormalities in Patients Undergoing Microlaryngeal Surgery in a Tertiary Care Hospital

The study aimed at investigating the impact of demographic, etiological, and risk factors on vocal fold lesions. Specifically, it involved analyzing and categorizing both benign and malignant lesions in patients who require microlaryngeal surgery.
Case PresentationAbout 100 participants, who were scheduled to undergo microlaryngeal study for vocal fold abnormalities at IMS and SUM Hospital between September 2022 and March 2024 were analyzed the following inclusion and exclusion criteria. The study explored the correlation between histopathological diagnoses and clinical features observed during laryngeal endoscopy. Most cases were seen in individuals aged 30–50 years, with a predominance of male patients, 84%. Smoking was strongly associated with both benign and malignant lesions, with alcohol use further increasing the risk for malignancy. Acid reflux was noted in 32% of both benign and malignant cases, suggesting a potential link to malignancy. Hoarseness of voice was the most common symptom with 90% patients, while difficulty in breathing was significantly associated with malignant tumors (p < 0.01).
ConclusionThe key observations from the study with microlaryngeal surgery are that most patients aged 31–50; and the male: female ratio 5:1, belonging any diverse profession with acid reflux. However, smoking linked to malignant lesions at p < 0.05. Furthermore, hoarseness more frequently, while dyspnea was significantly associated with malignancy at p < 0.05. With minor occurrence of nasal polyps, benign tumours 58% was seen frequently than > malignant ones 26%. The study underscores that while clinical laryngoscopy is essential for initial evaluation, it is not definitive. Histopathological examination (HPE) remains the gold standard for accurately diagnosing vocal fold abnormalities. Microlaryngeal surgery is crucial not only for therapeutic excision but, more importantly, for obtaining tissue for a definitive diagnosis, which is fundamental for guiding appropriate treatment and determining patient prognosis.
