Background <p>Intrahepatic cholangiocarcinoma (CCA) is considered the second most common type of liver cancer. As the survival rates of patients have significantly improved after successful tumour removal due to advances in adjuvant chemotherapy, regular monitoring has become crucial for identifying recurrence. In this article, we present an atypical recurrence of CCA that showed peculiar imaging characteristics, emphasizing the role of triphasic computerized tomography (CT) in the follow-up of such neoplasms.</p> Case Presentation <p>A 44-year-old male came for follow-up after left hepatectomy for a left hepatic duct mass-forming CCA. Triphasic CT revealed a newly developed non-mass enhancement surrounding the right posterior hepatic duct. Recurrence was suspected, and the patient underwent an ultrasound scan (US) and a tru-cut biopsy from the lesion that confirmed the recurrence.</p> Conclusions <p>When monitoring for potential recurrence of intrahepatic CCA, the radiologist should be vigilant for any newly developed areas of abnormal enhancement, regardless of the pattern. Despite the characteristic delayed enhancement of CCA, other peculiar enhancement forms may still be observed, particularly for the non-mass forming type. A single-phase contrast-enhanced CT scan is insufficient for surveilling biliary tract tumours following successful resection.</p>
