<p>This study aimed to determine the frequency of dyslipidaemia among ischemic stroke patients presenting to tertiary care Khyber Teaching Hospital, Peshawar. A cross-sectional study was conducted from 15 October 2022 to 14 April 2023, including 203 ischemic stroke patients. Dyslipidaemia was assessed using fasting lipid profiles and defined based on NCEP ATP III criteria. Data were analysed using SPSS version 25.0, and associations were evaluated with chi-square tests. A <i>p</i>-value of &lt; 0.05 was considered statistically significant. Among 203 patients, 67.0% had dyslipidaemia. Elevated LDL levels were observed in 48.7%, elevated triglycerides in 58.1%, and elevated total cholesterol in 45.3%, while low HDL was noted in 62.4% of patients. Dyslipidaemia prevalence was higher in males (<i>p</i> = 0.023), with 74.5% of dyslipidaemia patients being overweight or obese (<i>p</i> = 0.008). Comorbid conditions, including hypertension (81.6%) and diabetes mellitus (52.2%), were significantly associated with dyslipidaemia (<i>p</i> = 0.016 and <i>p</i> = 0.024, respectively). Dyslipidaemia is highly prevalent among ischemic stroke patients, with significant associations with gender, BMI, and comorbid conditions. These findings underscore the need for early identification and management of dyslipidaemia to prevent stroke and improve outcomes in rural populations.</p>
