Screening Healthcare Providers for Sleep Disturbance: Validity and Cutoff Score for the PHQ-9 Sleep Item

The length of most sleep disturbance instruments is a main reason that precludes prohibits their use in both research and clinical settings. The main purpose of the current paper is to evaluate the validity of a single sleep item and propose a cutoff score. Data from 238 healthcare providers (54.2% nurses and 45.8% physicians) was used in this study. The convergent and divergent validity of the PHQ-Sleep item was evaluated using two valid and reliable sleep-related scales. The PHQ-Sleep item correlated significantly positively with the Insomnia Severity Index (r = .69) but significantly negatively with the Sleep Quality Scale (r = –.59). A cutoff score of ≥ 15 on the Insomnia Severity Index was associated with a cutoff value of ≥ 2 (more than half the days) on the PHQ-Sleep item. The area under the curve was .84 (95% CI: .771—.909) with a sensitivity of 71.87% and a specificity of 84.47%. The results of this study add to growing evidence of the effectiveness of the PHQ-Sleep item as a valuable single item measure for sleep disturbance in both research and clinical settings. Notwithstanding the contributions of this study to sleep literature, the results should be interpreted in the context of its limitations, including its sample size and the selection process.
