<p>Paraquat is a contact herbicide widely used in some countries and often causes fatality when consumed in lethal doses. A rare phenomenon associated with paraquat poisoning is spontaneous pneumomediastinum which is associated with a high case fatality rate. We present 03 cases of paraquat poisoning who had developed spontaneous mediastinum. We managed cases of severe paraquat poisoning with pneumomediastinum as per existing evidence. Despite aggressive management, all the cases had adverse outcomes. Spontaneous pneumomediastinum is a potentially fatal complication of paraquat poisoning. This case series highlights the different courses of illness experienced by different patients after ingestion of paraquat in high doses and stresses on the importance of preventing the consumption of paraquat till an effective antidote is discovered.</p>
