Diagnostic Yield of Small Bowel Evaluation Techniques: A Comparative Study of Capsule Endoscopy, Push Enteroscopy, and Balloon Enteroscopy in Gastrointestinal Bleeding

Gastrointestinal bleeding (GIB) is one of the common causes of hospitalization. Obscure gastrointestinal bleeding (OGIB) is an indication for implementing various diagnostic methods to find the cause. The purpose of this study is to investigate and compare the findings of various methods for evaluating the small intestine, including push enteroscopy, retrograde push enteroscopy, capsule endoscopy, and single-balloon enteroscopy. In this observational-analytical cohort study, patients who presented with symptoms or suspicion of GIB between the years 2017 to 2023 and were referred to the endoscopy or gastroenterology department of Namazi Hospital in Shiraz, Iran, and were evaluated as having OGIB were examined. In total, 244 patients were enrolled in the study, of whom 94 cases (38.52%) were female and the remainder were male. The age range of the patients varied from 17 to 104 years, with a mean age of 50.59 (standard deviation of 14.61). Overt bleeding (in the form of melena, hematochezia, or hematemesis), iron deficiency anemia, and abdominal pain were among the most common clinical complaints. More than half of the patients had normal findings on their examinations using the four mentioned methods; however, ulcers/erosions (2.26%) and vascular ectasia (8.9%) were among the most common positive findings. The diagnostic yield of capsule endoscopy was higher (75.8%) compared to other methods, with single-balloon enteroscopy, push enteroscopy, and retrograde push enteroscopy following in subsequent ranks. This study is the first of its kind in Iran to investigate these four methods in patients with gastrointestinal bleeding. The findings of this study indicate that capsule endoscopy identifies a higher percentage of lesions in patients compared to other methods, highlighting the importance of this approach in the diagnostic and therapeutic process for patients.
