<p>Medulloblastomas are considered the most common pediatric tumors. Although they are more likely to present metastases, unlike other central nervous system primary tumors, the dissemination site generally occurs at the leptomeningeal spinal level, with extracranial metastasis extremely rare. A systematic review of metastatic medulloblastomas reported in the literature from 1963 to 2024 was performed under the PRISMA recommendations in PubMed and Web of Science. After the search filters, 23 articles were selected for the final review. In addition, we present two cases of atypical metastasis in the location of medulloblastomas: one patient with intraparotid lymphatic and sternum metastasis and another with skin metastatic lesions of medulloblastoma. These findings are of significant importance due to their rarity and unique presentations. To date, no case has been reported where both sites of medulloblastoma metastasis affect the same patient, and the intraparotid lymphatic location has not been recognized in the literature or soft tissue in adult patients.</p>
