<p>Maculopathies are ocular disorders increasingly prevalent in the elderly population, with numerous equally effective therapeutic alternatives, including biosimilar drugs, which could result in significant resource savings. This study aimed to highlight the high cost incurred in the purchase of anti-VEGF drugs in light of the increasing number of patients and new drugs placed on the market, as well as to raise awareness among healthcare professionals regarding the increased use of biosimilars. The number of patients divided by gender, the number of vials, and the corresponding expenditure on anti-VEGF drugs used in the treatment of maculopathies have been extracted and compared in two Italian healthcare companies analyzing the second half of 2022 and 2023. In both healthcare companies, there was an increase in treatments and healthcare expenditure during the periods considered, with 670 patients and an expenditure of €626,602.75 at Policlinico Umberto I, whereas at ASL Napoli 3 SUD, the number rose to 767 patients for a total expenditure of €715,630.22. Considering the high costs of maculopathy therapy, central governments and regulatory agencies should increasingly promote pharmaceutical monitoring actions and deliberate guidelines in favor of greater therapeutic appropriateness with the use of biosimilar drugs, which guarantee, with equal clinical efficacy, greater healthcare savings.</p>
