<p>Scrub typhus is a re-emerging infection with a course which can range from moderate to complicated leading to multi-organ failure and a high fatality of 30–40%. There is a significant knowledge gap about this neglected illness in healthcare personnel. Any patient presenting as acute undifferentiated febrile illness and associated with organ involvement should be investigated for scrub typhus especially in endemic areas. A high clinical suspicion and timely management of such a deadly disease is an absolute necessity. Here, we report the rarest presentation of scrub typhus in a case of hysterectomy with sepsis finally having multiorgan failure and a fatal outcome after a prolonged hospital course. Scrub typhus should always be included in the differential diagnosis of patients presenting with acute undifferentiated febrile illness even in the absence of characteristic eschar, especially in an endemic area. Early diagnosis and management can prevent multiorgan involvement and result in a favourable outcome.</p>
