<p>Rice stem rot (RSR) is a devastating fungal disease that poses a major challenge to rice production, causing considerable yield reductions in the North-Western region (NWR) of Haryana, India. To mitigate these losses, the present study aimed to assess the effectiveness of systemic fungicides and biorationals approaches in managing RSR during the <i>kharif</i> seasons of 2022 and 2023. Field trials were carried out at CCSHAU, Rice Research Station, Kaul, using a randomized block design with four replications. Among the three fungicides tested, hexaconazole 5% EC, proved most effective, reducing RSR severity by 76.15%. Among the botanical extracts and bioagents, garlic extract and <i>Trichoderma harzianum</i>, significantly reduced RSR severity by 42.20% and 26.44%, respectively, compared to the untreated control. All treatments led to significant enhancement in grain yield in both seasons, with hexaconazole 5% EC recorded the highest yield (7357.66&#xa0;kg/ha in <i>kharif</i> 2023; 7193.84&#xa0;kg/ha in <i>kharif</i> 2022) among fungicidal treatments, while in phytoextracts and bioagents, garlic extract and <i>T. harzianum</i> provided grain yield of 5898.25&#xa0;kg/ha in <i>kharif</i> 2023 and 5836.79&#xa0;kg/ha in <i>kharif</i> 2022, respectively. Greatest increase in grain yield was observed in <i>kharif</i> 2023 was associated with less rainfall (108.80&#xa0;mm) and slightly lower relative humidity (93.40%) compared to <i>kharif</i> 2022. These findings highlight hexaconazole, garlic extract and <i>T. harzianum</i> as promising approaches for RSR management, offering a combination of chemical and biorationals that could be integrated for sustainable disease suppression and improving grain yield in the NWR of Haryana, India.</p>
