Sexual health is a central component of radiation oncology care. After pelvic irradiation for gynecologic tumors, vaginal changes frequently occur that can impair sexuality and intimacy. These include epithelial atrophy, vaginal dryness, telangiectasias, adhesions, and vaginal stenosis, with their extent determined by individual tissue response and radiation dose. Recommended measures include water-based lubricants, vaginal moisturizers, and local estrogen substitution in patients with non-hormone-sensitive tumors. To prevent adhesions, scar formation, and stenosis, consistent vaginal care and regular stretching are advised, usually with dilators. Vibrators can represent a more acceptable alternative, as they support stretching, blood flow, and sensory activation. A decline in sexual interest is common and usually reversible. Mindful self-exploration, open communication, and, if needed, sexual therapy can support the restoration of bodily confidence and intimacy.
