<p>The only proven treatment for celiac disease is lifelong adherence to a&#xa0;strict gluten-free diet. However, complete avoidance of gluten is difficult, and many patients continue to experience symptoms or histological abnormalities despite their best efforts. Given the high prevalence of celiac disease and its well-defined pathophysiology, drug development has become an increasingly attractive approach to address the limitations of dietary management. Current therapeutic strategies primarily target nonresponsive celiac disease-persistent symptoms or disease activity despite a&#xa0;gluten-free diet—as well as the more severe refractory celiac disease types&#xa0;I and II. Several treatments aimed at specific disease mechanisms have advanced to phase&#xa0;2 and&#xa0;3 clinical trials. This review explores these emerging therapies in terms of efficacy, practicality and safety, and the needs of patients.</p>
