Photobiomodulation in the treatment of idiopathic facial paralysis: a case report and literature review

The aim of this study is to evaluate the efficacy of photobiomodulation (PBM) in the treatment of idiopathic facial paralysis, also known as Bell’s palsy (BP).
BackgroundBP is an acute, idiopathic disorder caused by a peripheral lesion of the facial nerve, resulting in paresis or total paralysis of the facial muscles, thereby compromising function and aesthetics. PBM has demonstrated efficacy in nerve regeneration and the functional recovery of peripheral nerves, representing a non-invasive therapeutic option with no side effects.
Case reportA 50-year-old man presented with facial paralysis on the left side for 40 days, with difficulty in phonation and chewing. On physical examination, facial asymmetry, facial drooping and absence of facial movement on the left side were noted, including an inability to close the left eye and arching of the left eyebrow. The diagnosis was Bell’s palsy grade V (severe), according to the House-Brackmann Scale. The patient was treated with ten sessions of PBM using an infrared laser with a wavelength of 808 nm and a power of 100 mW. The laser was applied in continuous emission mode at 35 points along the left facial nerve path with 3 to 6 J per point. The patient noticed a 90% improvement in facial movements and facial symmetry, with the reestablishment of speech and chewing.
ConclusionPBM was effective in the treatment of BP in this case, proving to be a promising, noninvasive and safe therapeutic option, although a definitive protocol has yet to be established.
