Purpose <p>Evaluating the antibacterial effect and the smear layer removal efficiency of guava leaves extract alone and combined with diode laser irradiation.</p> Methods <p>Extracted human maxillary central incisors were decapitated at cementoenamel junction, followed by mechanical preparation of root canals with standardized working length, then inoculated by E. faecalis for 48 h. Prepared roots were divided into 6 groups according to the irrigation protocol, Group A: negative control (distilled water), Group B: positive control (sodium hypochlorite (NaOCl) 5.25%), Group C: NaOCl followed by diode laser 980 nm, Group D: extract of guava leaves (EGL), Group E: EGL followed by diode laser, Group F: EGL followed by NaOCl then diode laser. Bacterial samples were retrieved from roots of all groups before and after irrigation using sterile paper points. Infected paper points were cultured in Brain heart infusion broth. Comparison of bacterial count before and after irrigation was performed by Paired t-test, comparison between all groups was performed by ANOVA test. Scanning electron microscope was used to evaluate smear layer removal according to a scoring system.</p> Results <p>Group F showed the highest percentage of E. faecalis reduction (99% bacterial reduction) (<i>p</i> &lt; 0.05) and the most efficient removal of smear layer (<i>p</i> = 0.0001). The lowest bacterial reduction in treatment groups was observed in group D. Both groups B and D showed minimum smear layer removal (<i>p</i> = 0.0001).</p> Conclusion <p>EGL root canal irrigation followed by NaOCl then diode laser 980 nm irradiation showed significant maximum antibacterial (E.faecalis) effect and the highest smear layer removal.</p>
