Purpose <p>This study evaluated the effect of low-level laser therapy (LLLT) on pain resulting from interproximal stripping in orthodontic patients.</p> Methods <p>In this double-blind, parallel-group randomized controlled trial, 21 orthodontic patients requiring interproximal stripping were randomly assigned to three groups (<i>n</i> = 7 each): LLLT (940&#xa0;nm laser, 32&#xa0;J/cm², 180&#xa0;s, applied to labial and lingual surfaces of the anterior mandible), placebo (laser in off mode), and control (no laser). Pain levels were assessed using a visual analog scale (VAS) at 3, 24, and 72&#xa0;h post-treatment. Data were analyzed using SPSS with an alpha level of 0.05.</p> Results <p>At 3&#xa0;h post-treatment, patients in the LLLT group reported no pain (VAS = 0 at all time points). The placebo group experienced mild pain (VAS = 0.43 ± 0.53), while the control group reported significantly higher pain levels (VAS = 2.29 ± 0.76). Pain scores in the control group continued to decrease over time (VAS = 1.14 ± 0.90 at 3&#xa0;h, 0.57 ± 0.79 at 24&#xa0;h, and 0.14 ± 0.38 at 72&#xa0;h). A significant difference in pain scores was observed among the three groups after adjusting for demographics and oral hygiene (<i>P</i> &lt; 0.001). The LLLT and placebo groups had significantly lower mean pain scores compared to the control group (<i>P</i> &lt; 0.001), but there was no significant difference between the LLLT and placebo groups (<i>P</i> = 1.00).</p> Conclusion <p>LLLT effectively reduced pain associated with interproximal stripping in orthodontic patients.
