Purpose <p>Borderline ovarian tumors (BOTs) are atypical ovarian epithelial cell proliferation without stromal invasion. It accounts for 10%-20% of all epithelial ovarian tumors and is a major part of the general gynaecology and gynaeco-oncology OPD. There are multiple grey areas and a dearth of standard guidelines. The purpose of the study is to understand clinicopathological features of patients diagnosed with borderline ovarian tumours (BOT) at a tertiary cancer care centre.</p> Methods <p>A retrospective analysis of 27 patients diagnosed with BOT was conducted. Data were collected on demographics, tumor markers, surgical procedure, characteristics, stage, histopathological subtype, and follow-up from Sept 2019 to Sept 2024.</p> Results <p>The average age of patients diagnosed with BOT was 39.19 years. Tumour size averaged 14.14&#xa0;cm and BMI 23.77&#xa0;kg/m². The most common subtype was serous BOT (51.9%), followed by mucinous BOT (33.3%). The maximum number of patients presented in Stage IA was 66.7%. A microinvasive pattern was noted in 33.3% of cases. Fertility-sparing surgery was performed in 9 patients. Adnexal mass of &gt;5&#xa0;cm was noted in 11.1% of cases during the follow-up period.</p> Conclusion <p>Serous histology and early-stage presentation were common in BOT. Tumor markers did not follow any pattern. It was markedly high in few and normal in 1/3rd of cases. Clinical features, tumour marker profiles, frozen section and pathological clues may aid in providing tailor-made treatment.</p>
