Primary Leiomyosarcoma Arising from the Right Infundibulopelvic Ligament (Ovarian Pedicle): A Rare Retroperitoneal Tumor

Leiomyosarcomas are malignant smooth muscle tumors that rarely originate in the retroperitoneum and are extremely uncommon in relation to the infundibulopelvic ligament (ovarian pedicle). We report the case of a 68 year old female who presented with right lower abdominal pain radiating to the back. Imaging revealed a retroperitoneal mass abutting vital structures with preserved fat planes. Core biopsy suggested leiomyosarcoma, and the patient underwent complete surgical excision with right ureteric stenting and lymphadenectomy. Histopathology confirmed a conventional leiomyosarcoma (FNCLCC grade 2, pT2N0). Given the complete R0 resection, intermediate grade and absence of metastases, adjuvant therapy was withheld, and the patient remains disease free at 2 years of follow up. This report highlights the clinical, radiologic, pathologic, and therapeutic considerations of this exceedingly rare tumor, with an emphasis on diagnostic differentiation and management strategies supported by current literature.
