Clinical Outcomes of Neuroendocrine Carcinoma of the Cervix: Institutional Audit

To assess the treatment related outcomes of patients with neuroendocrine carcinoma (NECC) of the uterine cervix and their patterns of failure.
MethodsThis is a retrospective study in which patients with NECC FIGO 2009 stage I–IVA, who were treated between 2009 and 2021 at our institution were included. Patients were categorized as early (Stage I–IIA) and locally advanced stage (Stage IIB–IVA). The overall survival (OS) and patterns of relapse were studied. Univariate analysis was run to understand the effect of tumour and treatment related parameters on OS.
ResultsFifty-six (56) patients who were treated with radical intent were included. On immunohistochemistry, synaptophysin, chromogranin, CD 56 and neuron specific enolase expression was observed in 44/54 (81.5%), 29/53 (55%), 17/22 (77%) and 2/3 (67%) patients respectively. Thirteen patients presented with early stage disease and 43 patients presented with locally advanced stage disease. The 5-year OS for entire cohort was 34.5%, and for early and locally advanced NECC, the 5-year OS was 68.4% and 24.2% respectively. Twenty-six patients (46%) relapsed, of which the most common form of relapse was distant, occurring in 19/26 patients (73%). The most common site of distant relapse was liver, followed by lung and bones.
ConclusionPatients with NECC have poor prognosis and multimodality treatment is recommended. Prospective multi centric rare tumour registries will be beneficial in assessing long term outcomes of patients with this rare histology, especially with differential IHC expression in tumours.
