Objective <p>To present a rare case of colossal-sized hydrometra in a postmenopausal woman with vaginal stenosis, diagnosed as superficial spreading squamous cell carcinoma (SCC) of the cervix, and discuss its implications for diagnosis and treatment.</p> Case Presentation <p>An 80-year-old postmenopausal woman with abdominal distension was found to have a large abdominopelvic mass. Imaging revealed a massive endometrial collection. Vaginal stenosis prevented internal pelvic examination. Emergency laparotomy revealed a distended uterus with approximately 5&#xa0;L of watery fluid. Histopathology post-hysterectomy confirmed poorly differentiated, HPV-associated superficially spreading SCC of the cervix. The patient recovered uneventfully and received adjuvant chemoradiation, with no recurrence at 6-month follow-up.</p> Conclusion <p>This case highlights the association between hydrometra and superficially spreading cervical SCC in postmenopausal women. Senile cervicovaginal stenosis may facilitate superficial cervical cancer spread, emphasizing the importance of histopathological examination of hysterectomy specimens when biopsy is impractical. Further research is needed to establish clinical guidelines.</p>

Objective
