Undifferentiated Uterine Sarcoma Causing Non-puerperal Uterine Inversion: A case report

Non-puerperal uterine inversion is a rare clinical scenario with aetiology ranging from benign causes to malignancies like endometrial cancers and uterine sarcomas. Diagnosis and management of uterine inversion in a non-puerperal setting pose challenges to surgeons due to distorted anatomical landmarks.

We describe a case of undifferentiated uterine sarcoma causing uterine inversion in a postmenopausal patient. The patient presented with bleeding per vaginum and clinical examination showed a necrotic mass filling vagina. Biopsy from the mass showed undifferentiated neoplasm. The patient was managed surgically with a hysterectomy with bilateral salpingo-oophorectomy and sampling of pelvic and paraaortic nodes.

Management of non-puerperal uterine inversion should be individualized based on aetiology. Surgical complexity due to distorted anatomy may lead to iatrogenic injury of surrounding structures. A high index of clinical suspicion and proper planning can help optimize the outcomes of such patients.
