While luminal A breast cancer often has a favorable prognosis, omitting radiotherapy (RT) after breast-conserving surgery (BCS) remains a controversial decision. This article addresses arguments against RT omission, focusing on the risks of local recurrence, quality of life (QoL) considerations, and the benefits of modern RT techniques. We emphasize the need for careful patient selection and ongoing research to refine risk stratification and ensure optimal outcomes.
