Purpose <p>Concerns have emerged about the potential tumor risks associated with computed tomography (CT) scans, particularly in pediatric patients who are more radiosensitive. This study aimed to evaluate the potential association between pediatric abdominal and pelvic CT scans and the subsequent risk of developing malignant and benign genital tumors.</p> Methods <p>From 2009 to 2019, pediatric patients (&lt; 18 years) who underwent abdominal and pelvic CT scans were identified using data from the Health and Welfare Data Science Center (HWDC) and extracted from the Taiwan National Health Insurance Research Database (NHIRD). The study included 22,926 individuals (mean age: 11.4 years), followed for an average of 7.48 years.</p> Results <p>The analysis demonstrated that patients who underwent abdominal and pelvic CT scans had a significantly increased risk of developing genital tumors compared with those who did not undergo such scans, with the association being particularly pronounced in females (adjusted hazard ratio [aHR] = 2.89, <i>p</i> &lt; 0.001). After excluding patients who developed tumors within the first two years of follow-up, the effect remained significant in females. The increased risk was driven primarily by benign tumors (aHR = 1.99, <i>p</i> &lt; 0.001).</p> Conclusion <p>Pediatric abdominal and pelvic CT scans are associated with an increased risk of genital tumors, particularly in females, with most of the excess risk attributable to benign tumors. Although these tumors are non-malignant, their potential clinical and psychological impacts highlight the need to minimize CT radiation dose in pediatric populations.</p>
