Sensitization patterns to molecular allergens of house dust mites (Dermatophagoides pteronyssinus) on the Canary Islands

This study aimed to characterize the molecular sensitization patterns to Dermatophagoides pteronyssinus (Der p) allergens in allergic patients from the Canary Islands, a region with high house dust mite (HDM) exposure. We sought to identify predominant Der p components, examine age-related variations, and explore phenotypic clustering to inform region-specific allergy diagnostics and therapy.
MethodsA retrospective, cross-sectional analysis was conducted on 1685 patients with suspected allergic disease who underwent molecular allergy testing using the ALEX2® multiplex assay between 2021 and 2025. Specific immunoglobulin E (IgE) sensitization (≥ 0.35 kU/L) to nine Der p components was assessed. Patients were stratified by age, and statistical analyses included Kruskal–Wallis tests, χ2 tests, Spearman correlation, principal component analysis (PCA), and k‑means clustering to identify molecular profiles.
ResultsOverall, 62.3% of patients were sensitized to at least one Der p component. The most prevalent allergens were Der p 20 (46.4%), Der p 7 (45.9%), and Der p 1 (45.7%), while Der p 2 was notably low (7.3%). Adolescents showed the highest sensitization rates and IgE levels. Total IgE correlated positively with the number of sensitizations (ρ = 0.50, p < 0.0001). Cluster analysis revealed three distinct molecular phenotypes with differing age distributions and allergenic loads.
ConclusionThe molecular sensitization profile for individuals on the Canary Islands deviates from typical European patterns, with low Der p 2 and high Der p 7/20 recognition. These findings underscore the need for region-specific diagnostics and personalized immunotherapy targeting relevant molecular allergens.
