<p>Advances in oncology have significantly improved survival, leading to a growing population of cancer survivors. In this setting, endocrine and metabolic complications represent common long-term sequelae, with a substantial impact on quality of life and overall prognosis. Their clinical presentation varies according to tumour type and treatment modalities: immunotherapy is associated with immune-related endocrinopathies, androgen deprivation therapy in prostate cancer drives metabolic and skeletal alterations, while endocrine therapies and cardiotoxic treatments in breast cancer contribute to increased cardiovascular risk and bone loss. In colorectal cancer, endocrine sequelae are less specific and mainly related to systemic treatment effects and shared risk factors. Optimal management requires a multidisciplinary approach based on active surveillance, prevention strategies, and early treatment of complications.</p>
