Prolactin contributes to metabolic adaptation during pregnancy by modulating beta-cell function and glucose homeostasis. Clinical evidence suggests a non-linear and time-dependent relationship with gestational diabetes mellitus, in which both reduced levels in late gestation and alterations in hormonal dynamics may be associated with metabolic dysfunction. Large-scale prospective studies are needed to define its value as an early biomarker of impaired glucose metabolism.
