Cushing’s Syndrome during pregnancy is a rare and challenging condition, associated with significant maternal and foetal morbidity. Diagnosis is complicated by physiological changes in the hypothalamic-pituitary-adrenal (HPA) axis, requiring particular care in interpreting biochemical data. Surgery, in the rare cases where it is necessary and feasible, is preferable during the second trimester, while specific precautions are required for the medical treatment when indicated. Management should be multidisciplinary. In patients with reproductive desire, preconception planning is necessary in order to optimise maternal and foetal outcomes.
