<p>Diabetes mellitus is rapidly increasing worldwide, and insulin therapy represents a cornerstone of treatment for type 1 diabetes and many cases of type 2 diabetes. However, many people living with diabetes face challenges in achieving optimal glycaemic control, often due to the complexity of insulin treatment and fear of experiencing hypoglycaemic episodes. In recent years, the introduction of innovative technologies has significantly improved diabetes management, enhancing therapeutic adherence and promoting patient self-management. In particular, connected insulin pens and caps automatically record injection dose, date, and time, transmitting this information to digital platforms or dedicated apps. Thus, connected pens enable simpler and more accurate monitoring of therapy, and some also provide tangible daily management support through missed-dose reminders, automatic calculation of mealtime and correction boluses, and alerts regarding pen temperature and insulin duration. Recent studies highlight that using these tools significantly increases time in range, improves adherence to insulin therapy, and enhances overall patient satisfaction compared to traditional methods. Furthermore, to facilitate basal insulin titration, often hindered by limited knowledge or concerns about hypoglycaemia risk, apps have been developed to suggest daily insulin dosage adjustments based on individual glucose readings and clinician-defined algorithms. Clinical studies demonstrate that these tools significantly improve glycaemic control compared to traditional titration methods, promoting greater patient autonomy and reducing hypoglycaemic episodes. The imminent introduction of novel insulin formulations, such as once-weekly basal insulins, will represent another significant advancement in diabetes management, presenting new challenges in proper insulin titration and therapeutic monitoring, areas where digital solutions may play an even more crucial role. The integration of these innovative technologies into everyday clinical practice can substantially simplify insulin management, improve therapy adherence and glycaemic control, and ultimately contribute to a better quality of life for people living with diabetes.</p>
