<p>Fragility fractures are a well-known complication of diabetes mellitus, significantly increasing patient morbidity and mortality. In patients with type 1 diabetes (T1DM), reduced bone mineral density (BMD) increases the risk of fractures up to sixfold compared to the general population. In contrast, in type 2 diabetes (T2DM), fracture risk is increased despite normal or even higher BMD. This suggests that additional factors, such as alterations in bone material properties and bone microarchitecture, may play a crucial role in mediating skeletal fragility in T2DM. This review aims to discuss and highlight the most recent studies on the pathophysiological mechanisms underlying bone fragility in diabetes, as well as emerging treatment strategies to preserve bone health.</p>
