The definition of Non-Invasive Follicular Thyroid neoplasm with Papillary nuclear features (NIFTP), in addition to the nomenclature change has implied the declassification of these lesions from malignant to benign. At sonography, NIFTP resembles benign lesions or lesions at low risk of malignancy, often with indeterminate cytology. By definition, NIFTP cannot be BRAFV600E-mutated; therefore, the search for BRAF mutations is necessary to exclude the diagnosis. Strict compliance with the diagnostic criteria (absence of infiltration of the tumour capsule, absence of BRAF mutations, absence of papillary structures) is essential to avoid diagnostic errors and the unexpected finding of metastases. Once it is certain that it is an NIFTP, postoperative management should be similar to that of benign lesions.
