Hirata’s disease is a rare form of endogenous hyperinsulinaemic hypoglycaemia, characterised by anti-insulin autoantibodies production. The pathogenesis involves a combination of genetic predisposition and trigger elements such as drugs and viruses; some forms are idiopathic. Due to the rarity of the condition, the therapy is still under debate. The ultra-fractionated low carbohydrate diet, the use of immunosuppressants and plasmapheresis can be employed to manage hypoglycaemia and control symptoms.
