Hypothalamic obesity, caused by acquired lesions or congenital dysfunctions of the neuronal centres involved in the control of caloric intake, has been a therapeutic challenge for decades, as it involves early weight gain and prominent hyperphagia. The most recent understanding of the mechanisms underlying weight gain already allows us to glimpse in the near future the potential of new pharmacological classes as a valid alternative to caloric restriction and bariatric surgery.
