Introduction <p>Tobacco and alcohol use are leading causes of preventable morbidity and mortality worldwide. In India, these behaviours remain highly prevalent among men, varying widely across states and regions. Understanding how different products interact and cluster spatially within states such as Tamil Nadu is essential for designing effective interventions. This study examined regional variations and structural interconnections in substance use through integrated spatial and social network analyses.</p> Methodology <p>Secondary data from the National Family Health Survey (NFHS-5, 2019–21) for Tamil Nadu were analyzed. Spatial analysis using Moran’s I and Local Indicators of Spatial Association (LISA) identified districts with high-high (hotspot) and low-low (coldspot) clustering of tobacco use. Separate social network analyses were then conducted for hotspot and coldspot regions using standardized co-use adjacency matrices for fifteen tobacco and alcohol products. Weighted degree, closeness, and betweenness centralities were calculated to identify the most influential products in each network.</p> Results <p>Spatial analysis revealed significant clustering of substance use across Tamil Nadu. In hotspot regions, cigarettes had the highest centrality, followed by hard liquor and beer, while in coldspot regions, hard liquor was most influential. Despite regional differences, these products consistently occupied central network positions, indicating a stable structural linkage between smoking and alcohol use.</p> Conclusion <p>Substance co-use among men in Tamil Nadu centres on cigarettes, hard liquor, and beer. Policies that reduce their affordability, availability, and accessibility through integrated taxation, retail regulation, behavioural interventions, and enhanced cessation support could effectively disrupt co-use patterns and lower overall prevalence.</p>
