A Review on Non-invasive Management of Early Carious Lesions Through Modern Approaches

This review aims to provide a comprehensive overview of modern, non-invasive approaches for managing early carious lesions, emphasizing the shift from invasive restorative techniques to preventive, minimally invasive strategies focusing on early detection and intervention to reduce the global burden of dental caries.
Recent FindingsA search in Medline and PubMed was conducted using the keywords: “Non-invasive” OR “early carious lesions” OR “enamel remineralization” OR “regeneration” OR “caries arrest,” following the Problem, Intervention, Comparator, Outcome (PICO) strategy to address the research question: “Which non-invasive treatment approach available for managing early carious lesions is effective?” The inclusion criteria defined appropriate randomized clinical trials (RCTs), meta-analyses, reviews, and systematic reviews that detail various treatment modalities or compare one treatment to another for managing early carious lesions found in the Medline and PubMed search between 1980 and 2024, including only publications available in the English language. The search yielded 370,134 results within the timeline. After removing irrelevant papers not related to the search, a total of 62 relevant publications were included. Recent studies have highlighted the effectiveness of various non-invasive treatments, including fluoride varnish, dental sealants, silver diamine fluoride (SDF), casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), and the self-assembling peptide P11-4. Notably, self-assembling peptide P11-4 has demonstrated significant potential in remineralizing early carious lesions by forming a 3D scaffold within demineralized enamel, which facilitates enamel regeneration. Clinical trials and meta-analyses have shown the superior outcomes of P11-4 compared to traditional fluoride varnish, positioning it as a promising option for non-invasive caries prevention management.
