Introduction <p>Incidental breast findings on PET imaging, though uncommon, are increasingly reported with the expanding use of non-[<sup>18</sup>F]FDG radiotracers in various oncologic settings. This pictorial essay illustrates a range of such lesions, emphasizing potential diagnostic pitfalls and highlighting the importance of accurate interpretation in clinical practice.</p> Methods <p>We collected clinical BC cases from two high-volume PET/CT clinical centres. Each centre checked its database to identify breast lesions detected on PET/CT performed with tracers different from [<sup>18</sup>F]FDG. Afterward, the chosen cases were reviewed collectively by the authors, with emphasis placed on those considered to have significant clinical relevance.</p> Results <p>Following the screening process, four clinical cases were identified. The selected cases demonstrate a diverse range of benign and malignant findings on [<sup>11</sup>C]methionine, [<sup>18</sup>F]PSMA-1007, [<sup>68</sup>Ga]Ga-Pentixafor and [<sup>68</sup>Ga]Ga-DOTATOC, including a suspicious uptake (further follow-up is needed), a myofibroblastoma, a bifocal invasive ductal carcinoma of no special type, and a case of benign breast lesion with fibrocystic changes.</p> Conclusions <p>These cases highlight the diverse PET imaging appearances of breast lesions detected with non-[<sup>18</sup>F]FDG tracers. Imaging and histopathological correlations are essential to avoid misdiagnosis.</p>

Introduction

Incidental breast findings on PET imaging, though uncommon, are increasingly reported with the expanding use of non-[18F]FDG radiotracers in various oncologic settings. This pictorial essay illustrates a range of such lesions, emphasizing potential diagnostic pitfalls and highlighting the importance of accurate interpretation in clinical practice.

Methods

We collected clinical BC cases from two high-volume PET/CT clinical centres. Each centre checked its database to identify breast lesions detected on PET/CT performed with tracers different from [18F]FDG.
