Value of extra-cardiac sympathetic nervous uptake parameters based on 123I-MIBG SPECT/CT in the assessment of Parkinson’s disease

To compare 123I-metaiodobenzylguanidine (MIBG) scintigraphy parameters in non-cardiac peripheral sympathetic organs of Parkinson’s disease (PD) patients and controls, and to assess its potential as a biomarker for PD, as well as its correlation with clinical parameters in PD patients.
MethodsWe recruited 63 PD patients and 48 non-PD controls [26 Parkinson-plus Syndrome (PPS) and 22 Essential Tremor] diagnosed between January 2021 and May 2024 at the Beijing Friendship Hospital. The MIBG uptake in the salivary and thyroid glands was evaluated using ratio parameters, and washout rates (WR) were assessed simultaneously. The diagnostic performance was analyzed using receiver operating characteristic (ROC) curves. Additionally, correlations between cardiac and peripheral scintigraphy parameters and with clinical variables were analyzed in PD patients.
ResultsNo significant differences in ratio parameters of the parotid, submandibular, and thyroid glands were found between PD patients and controls. However, the WR of the parotid and thyroid glands showed significant differences, with moderate diagnostic efficacy (AUC = 0.648 and 0.638, respectively). The WR of salivary and thyroid glands showed a significant negative correlation with the heart-to-mediastinum (H/M) ratio. Disease duration was significantly negatively correlated with ratio parameters of salivary and thyroid glands, while the Unified Parkinson’s Disease Rating Scale (UPDRS) total score correlated significantly only with the ratio parameters in the parotid gland. No significant correlation was observed between the Hoehn and Yahr (H–Y) stage and scintigraphy parameters.
ConclusionThe WR of parotid and thyroid glands can serve as biomarkers to assist in PD diagnosis. Using these parameters as auxiliary diagnostic tools in patients with concurrent cardiac conditions may offer substantial clinical value.
