<p>Antibiotics with activity against the widespread and difficult-to-treat metallo-β-lactamase (MBL)-producing Gram-negative bacteria are lacking, with treatment being further challenged by evolving antimicrobial resistance mechanisms. Cefiderocol monotherapy and a combination of aztreonam and avibactam (either as a single product formulation or through co-administration of ceftazidime/avibactam and aztreonam) are the preferred treatment options for infections caused by MBL-producing bacteria, including Enterobacterales, <i>Pseudomonas aeruginosa</i> and <i>Acinetobacter baumannii</i>, though their efficacy has been demonstrated in few clinical trials. Although new agents with activity against MBL-producers are in development, there is a need for further research to address the shortage of treatment options.</p>
