Purpose of Review <p>Current guidelines recommend moderate to vigorous-intensity exercise for people with spinal cord injuries (SCI). The objective of this paper is to specify these guidelines by comparing improvement in peak oxygen uptake (VO2peak) after high-intensity training to low/moderate-intensity training.</p> Recent Findings <p>From inception to July 2023, 6 randomized controlled trials and crossover studies comparing high-intensity exercise to low/moderate-intensity exercise on VO2peak improvement in people with SCI were selected from 1102 studies from PubMed, CINAHL, Scopus, and SPORTDiscus. The standardized mean difference in VO2peak improvement after high-intensity exercise and low/moderate-intensity exercise was 0.04, favoring high-intensity exercise with a 95% confidence interval, -0.37–0.45.</p> Summary <p>The improvement in VO2peak was nonsignificant, suggesting high-intensity exercise alone is not superior for cardiometabolic outcomes. This data supports current guidelines and increases flexibility for exercise selection. Further research is needed to compare low-intensity exercise to moderate-intensity exercise.</p>
