Comorbidity and Functional Outcomes After Hip Fracture: An Umbrella Review

Individuals who have suffered a hip fracture frequently have comorbid conditions that may influence functional outcomes. To better understand the association between comorbidity and functional outcomes after hip fracture we conducted an umbrella review (a synthesis of existing systematic reviews).
Recent FindingsMEDLINE (Ovid), Embase, and the Cochrane Database of Systematic Reviews were searched. Systematic reviews that include cohort studies published at any time and online ahead of print reporting on comorbidity and functional outcomes (I.e. FIM, BI, ambulation) after hip fracture were included in this umbrella review. The search identified 2787 studies of which four systematic reviews published between 2016 and 2020, captured a total of 151 studies. Using the JBI (Joanna Briggs Institute) Checklist for grading systematic review quality, 1 study was high quality, 2 were moderate quality and 1 was low quality. All reviews were cohesively in agreement that pre-fracture comorbidity burden is associated with poor functional outcome after hip fracture. The Charlson Comorbidity Index was the most common measure of comorbidity burden. Specific comorbidities such as cognitive impairment and depression were found to contribute to functional outcome. The Functional Independence Measure was common among a variety functional outcome measures.
SummaryThere is a general relationship between comorbidity and functional outcomes after hip fracture. However, there is wide variability in measures of comorbidity and specific functional outcomes utilized therefore complicating the relationship among these two factors after hip fracture.
