Ultrasound-Guided Releases of Common Upper Extremity Nerve and Tendon Entrapments

Purpose of Review

This report is a narrative review of the various ultrasound (US)-guided techniques used for releasing commonly entrapped upper extremity nerves and tendons.

Recent Findings

In recent years, there has been a notable surge in publications on US-guided techniques for releasing various anatomical structures. US serves as a valuable tool for identifying structures requiring release and assessing those at risk of injury during the procedure. The application of US-guided techniques presents distinct advantages over traditional surgical releases, such as decreased incision length and quicker recovery time. Current research on US-guided techniques for trigger finger and carpal tunnel release indicates high success rates of over 97% and 98%, respectively, along with a favorable safety profile. However, there is a notable gap in the literature concerning studies on first and sixth dorsal extensor compartment as well as cubital tunnel releases.

Summary

The existing literature demonstrates variable levels of evidence regarding the effectiveness of US-guided releases of anatomical structures in the upper extremity. Further research is needed to understand the outcomes associated with various instruments and techniques, as well as to compare them with traditional surgical methods. Addressing these gaps will aid in refining US-guided releases and shedding light on their utility for management of entrapped upper extremity nerves and tendons.
