Alternative Bone Flaps in Head and Neck Reconstruction

This review evaluates the use of various vascularized osteocutaneous flaps in head and neck reconstruction, comparing the fibula free flap (FFF) with alternative options: the deep circumflex iliac artery (DCIA) flap, scapular free flap (SFF), medial femoral condyle (MFC) and trochlea (MFT) flaps, and the osteocutaneous radial forearm flap (OCRFF). By analyzing recent data and findings, we aim to clarify the unique characteristics, benefits, drawbacks, and clinical applications of each flap type.
Recent FindingsThe role of alternative bone flaps beyond the FFF is expanding in head and neck reconstruction. Comparative studies and advances in surgical techniques have highlighted the benefits of these alternatives. For example, the DCIA flap and SFF offer enhanced soft tissue coverage and natural contouring, while the MFC and MFT flaps are suited for smaller, specialized defects. The OCRFF is particularly effective for small-area defects in patients with comorbidities or contraindications to other flap types.
