Purpose of Review <p>Telemedicine in the Pediatric Intensive Care Unit (PICU) represents a&#xa0;promising frontier for expanding pediatric intensive care, offering solutions to overcome&#xa0;disparities in access and quality of care. This review article outlines the vast potential of&#xa0;TelePICU and highlights the importance of understanding its current state to inform its future&#xa0;development.</p> Recent Findings <p>By significantly expanding access to specialists, regardless of&#xa0;geographical location, TelePICU optimizes resources, empowers remote teams, and directly&#xa0;contributes to reducing morbidity and mortality, as well as the length of stay, in critically ill&#xa0;patients. Modalities such as synchronous teleconsultations, continuous telemonitoring, and&#xa0;tele-education demonstrate the ability to elevate care standards and promote superior clinical&#xa0;outcomes. However, the full realization of these potentialities requires a critical analysis of&#xa0;existing challenges, such as the heterogeneity of technological infrastructure, ethical and&#xa0;regulatory complexities, and the need to strengthen professional acceptance. TelePICU's&#xa0;development is pointing towards deeper integration with artificial intelligence and big data,&#xa0;thereby expanding its diagnostic capabilities and decision support.</p> Summary <p>A&#xa0;comprehensive understanding of the current scenario, coupled with strategic investments in&#xa0;research, infrastructure, and regulation, is essential for shaping the future of TelePICU and&#xa0;consolidating it as a key pillar for equitable and high-quality pediatric intensive care.</p>
