The Significance of Serum Beta-Hydroxybutyrate and Urine Biochemical Markers in Determining the Severity of COVID-19 Infection

This study aims to assess the significance of biochemical indicators in urine and serum beta-hydroxybutyrate in determining the severity of COVID-19 infection. The study involved 49 participants who tested positive for COVID-19. Urine biochemical parameters were analyzed using an automated urine analyzer (OrinalyserU-500), and beta-hydroxybutyrate (β-OHB) levels in the blood were determined using a chemistry analyzer (Chem 7 ERBA serum chemistry analyzer, Erba Mannheim, Germany). In the study, a non-significant correlation was found between urine biochemical parameters and patients with COVID-19. Patients with moderate to severe disease exhibited significantly higher levels of urine protein, RBC concentration, and pH than those with mild infection (P < 0.05). However, urine glucose, ketones, albumin, and bilirubin showed non-significant increases in severe illness (P < 0.05). Serum BHB concentration and M.P were higher in moderate to severe disease than in mild infection (P < 0.05). No significant difference was observed between urine protein and MP levels in the severity of COVID-19 (P > 0.05). A significant difference was observed in urine protein levels between moderate and severe COVID-19 cases (P < 0.05). In contrast, significant differences in urine MP levels were observed between mild and moderate-severe cases (P < 0.05). Urine biochemical parameters, especially urine microprotein estimation, are not significantly correlated in severe SARS-CoV-2 patients. Monitoring proteinuria in COVID-19 patients is a valuable marker for acute kidney injury, an independent risk factor for mortality in ICU-admitted patients.
