Prevalence of Obstructive Sleep Apnea Hypopnea Syndrome among Patients with Interstitial Lung Disease

There has been an increase in understanding of the burden of sleep disordered breathing among patients with interstitial lung disease (ILD). This study aimed to compute the prevalence of obstructive sleep apnea hypopnea syndrome (OSAHS) among ILD patients without any traditional risk factors for OSAHS.
MethodsILD patients were recruited and underwent anthropometric evaluation, pulmonary function tests and overnight polysomnography. Sleep quality was assessed using Pittsburgh sleep quality index. Patients with body mass index > 30.0 kg/m2 and other conventional risk factors of OSAHS were excluded.
ResultsThirty patients constituted the study group with 11 males and 19 females. Mean age of population was 47.1 ± 11.8 years and median BMI was 24.1 kg/m2. Poor sleep quality was reported by 83.3% of ILD patients. Significant nocturnal oxygen desaturation was found in almost a third of patients. Median AHI and ODI were 10.2 and 10.6 events per hour with sleep efficiency of 76.0%. Prevalence of OSAHS was found to be 70.0% with 43.4% having moderate-to-severe OSAHS.
ConclusionsPoor sleep quality was common occurrence among patients of ILD. Significant proportion of them had OSAHS, despite the absence of conventional risk factors. High clinical suspicion of OSAHS must be kept while evaluating patients with ILD to improve their quality of life and long-term outcomes. Small sample size of the study limits generalizability, hence larger and diverse cohorts need to be analysed to enhance robustness of the conclusions.
