Background <p>Current challenges demand a&#xa0;change in the healthcare system. Primary care with community health nurses, which has long been part of everyday care in other countries, faces legal, structural and social obstacles in Germany.</p> Aim <p>The interest of this work is to explore the opportunities and challenges in the practical implementation of community health nursing (CHN) projects.</p> Methodology <p>To answer the research question “what opportunities and challenges arise in the implementation of community health nursing projects in Germany?”, a&#xa0;narrative expert interview was conducted with subsequent content analysis. Only one interview was conducted as only one person in this health center took on the role of CHN at the time of the survey. Based on this, the categories were systematically analyzed within the conceptual framework of the SWOT analysis, discussed with the co-author and recommendations for the practice were derived. The results were presented again to the interviewed expert prior to publication.</p> Results <p>The assignment of the categories formed within the SWOT analysis and the associated identification of opportunities and challenges in the implementation of CHN projects form the results section.</p> Conclusion <p>Support is needed at structural, social, political and legislative levels in order to create framework conditions that enable CHN.</p>
