Purpose of Review <p>An effective transition from inpatient to outpatient care is pivotal in ensuring successful recovery following hospitalization and preventing readmission. This review discusses potential discharge pitfalls for patients with skin disease and provides recent developments in management of this critical period.</p> Recent Findings <p>Standardized communication between dermatologists and primary teams, accurate discharge summaries, and patient education on discharge plans can improve adherence with dermatology recommendations at discharge. Management plans should be patient-centered and consider all aspects of care including therapeutics, supplies, and additional consultants. Teledermatology may improve attendance rates and accessibility of outpatient follow-up visits.</p> Summary <p>The transition to outpatient care is a complex process requiring multidisciplinary coordination and thoughtful planning prior to discharge. Dermatologists should actively participate in discharge planning and communication to ensure continuity of care for hospitalized patients with skin disease.</p>
