Treatment Approaches To Anxiety and Depression in Parkinson’s Disease

Anxiety and depression are common comorbidities in Parkinson’s Disease (PD) that manifest long before, or at any point during, the illness and worsen quality of life and overall prognosis. An area of active research, the complex relationship between psychiatric illness and Parkinson’s Disease suggests shared neurobiological underpinnings. Despite high comorbidity, psychiatric manifestations of PD remain underrecognized and undertreated, and available treatment options can be complicated to use. Medications for depression may worsen motor and autonomic symptoms and dopaminergic medications for motor manifestations have many potential psychiatric adverse effects. This complicated relationship demands a nuanced, interdisciplinary, evidence-based, and personalized approach. This review provides an update on the current literature for the diagnosis and management of anxiety and depression in Parkinson’s Disease.
Recent FindingsThere are several available pharmacologic and nonpharmacologic options for the treatment of anxiety and depression in PD. Many have limited evidence but can be appropriate options to alleviate mental suffering. In some cases, optimization of treatment for motor and autonomic symptoms of PD is the most appropriate intervention with benefits to mental health. Psychotherapy and exercise have robust bodies of evidence and should not be overlooked in a comprehensive treatment plan. Advances in neuromodulation techniques are an exciting new frontier in this space.
SummaryUnfortunately, limited evidence exists to guide the treatment of psychiatric symptoms in Parkinson’s Disease. Given the complex relationship between Parkinson’s Disease and psychiatric disease, usual algorithms for the treatment of depression and anxiety in late life differ for those with PD in important ways. All disciplines involved in the care of patients with PD should be mindful of the significant, negative impacts of untreated or undertreated psychiatric symptoms and follow an evidence-based approach to treatment as outlined in this review.
